I have several "Sperry" artificial horizons here, they are probably
similar. To un cage, normally you would need to pull and turn. If
nothing happens it might be corroded. The gyro will turn regardless of
caged or not.
Just to run the engine on the ground will not create enough vacuum to
run the gyro properly. It needs flying speed.
You can connect a vacuum cleaner to the instrument for bench testing.

As I remember the flag is mechanical and will only go away when the knob is
turned. Yes the gyro may turn but the horizon will remain stationary unless
the unit is un caged.
